Covered California

The Chinese Community Health Resource Center, in collaboration with the following organizations, will offer Mandarin, Cantonese, and English informational seminars on “Covered California.”  During the seminars, we will answer your questions, qualify you for Covered CA, and schedule an enrollment appointment at the enrollment sites listed below.

Please bring all of the following to the enrollment appointment:

• your government-issued photo ID
• social security number of each family member applying for Covered CA
• proof of income (most recent federal tax return, pay stubs, pension, rental/investment income,…etc.)
• proof of legal residency (passport, naturalization certificate, green card, work/student visa,… etc.)
• proof of address (utility bill, driver’s license)
• check book (payment for first month premium)

Please consider the following before your enrollment appointment:

• which primary care physician(s) and health plan for the individual/family. (For SF County: Blue Cross, Blue Shield, Chinese Community Health Plan, Health Net, & Kaiser)
• each individual/family has up to 4 coverage level options:  bronze, silver, gold, platinum – while platinum has the highest monthly premium, it pays 90% of covered health care expenses; bronze has the lowest monthly premium, but it pays only 60% of covered health expenses
